Brazosport Regional Home Health Receives TMF Award for Improvement in Management of Oral Medications

Brazosport Regional Home Health, a division of Brazosport Regional Health System, recently received bronze status for Improvement in Management of Oral Medications from TMF, the Medicare Quality Improvement Organization for Texas.
TMF held a quality initiative to improve publicly reported outcomes for home health agencies. The scoring was based on improvement in Acute Care Hospitalization percentage; either Improvement in Management of Oral Meds, Improvement in Dyspnea, or Improvement in Pain Interfering with Activity.
In order to participate, each home health agency was evaluated based on their agency data from Home Health Compare from February 2004-January 2005. Brazosport Regional Home Health decided to partake in the Improvement in Management of Oral Meds category. In November 2007, each participating agency was reevaluated and was awarded accordingly to their improvement scores.
Over 400 agencies participated in the initiative and set their goal of reducing failure rate by 18% with an ideal of 90%. Out of 400, 170 agencies chose to work on Improvement in Management of Oral Meds in Texas. Brazosport Regional Home Health achieved a bronze status of Improvement in Management of Oral Medications. They improved from a baseline of 21.21% and ended with a final score of 69.72% with a 70.5% achievement.
“To achieve this recognition, our nurses collaborated with the physicians to verify the medication list for our patients,” stated Wendy Charlesworth, Director of Brazosport Regional Home Health. “It was our mission to make sure we educated our patients on which medications to take after discharge. We are proud to have received this award and will continue to raise the bar in home healthcare.”
