Medical Records or formally known as Health Information Management (HIM) is responsible for cataloging and maintaining the medical records of each and every patient that is treated at Brazosport Regional Health System. Our staff ensures that the medical records of all patients are complete and that the information contained within is kept private.
RELEASE OF INFORMATION (ROI) SERVICES
ROI services provides health information to patients, physicians, other healthcare providers, attorneys, insurance companies and other parties with a need to obtain copies of patient records. With the introduction of the Health Insurance Portability and Accountability Act in 1996 (HIPAA) the rules and regulations regarding releasing Protected Health Information became more stringent and a great deal of expertise is required to comply with the laws. The Brazosport Regional HIM team includes a Certified Release of Information Specialist and others who are ready to assist you in obtaining your Protected Health Information. The Authorization to Release Protected Health Information should be filled out completely and mailed in or brought to the Health Information Management Department in person. Your photo identification is required to obtain copies of records. Please call the ROI desk with any questions.
PRIVACY OFFICER
The Director of HIM is Certified in Healthcare Privacy Compliance by the Compliance Certification Board and serves as the Privacy Officer for BRHS, investigating and resolving any complaints by patients of a breech of privacy of their Protected Health Information. She also handles all requests for an amendment to medical records in the case that a patient does not agree with the physician documentation on their chart.
BIRTH REGISTRY
The BRHS Birth Registry has received numerous awards for exemplary performance from the Texas Department of State Health Services including two top honors in 2010. The Attorney General of the State of Texas has also recognized BRHS birth registry for Outstanding Performance in the Paternity Opportunity Program in 2010. BRHS HIM has four Team Members who are certified by the Attorney General of the State of Texas to assist unmarried parents with the completion of Acknowledgement of Paternity documents. TUMOR REGISTRAR
Brazosport Regional HIM employs a Certified Tumor Registrar to collect and submit data on types and stages of cancer and results of treatment, including life long follow up, to the Texas Cancer Registry and the National Cancer Database (NCDB) since 1993. After survey in 2010 the program was deemed an accredited Community Hospital Cancer Program Facility of the American College of Surgeons (ACoS). The Registrar supports the activities of the Cancer Committee, Cancer Conference and the Cancer Program as a whole.
